COMMANDER FREDERICK L. Lewis u.s. NAVY COMMANDER CARRIER AIR WING EIGHT Commander Frederick L. Lewis was born in Los Ange- qu les, California on December 15, 1940. After graduating from l the United States Naval Academy with the class of 1962, he - 1 entered Flight Training at Naval Air Station, Pensacola, Flor- ida and was designated a Naval Aviator at Naval Air Station, Kingsville, Texas in November 1963. A one year tour as a Flight Instructor with Training Squad- ron TWO followed, after which he was ordered to Fighter ,- Squadron ONE TWO ONE as a Fleet Replacement Pilot. ' After completion of F-4 Pilot Training, Commander Lewis reported to Fighter Squadron SEVENTY-FOUR where he participated in the 1965-1966 Mediterranean deployment and 1967 Tonkin Gulf deployment in USS FORRESTAL CCVA 591. This deployment was abruptly terminated by a disastrous flight deck fire and Commander Lewis received - orders to Fighter Squadron THIRTY-THREE where he made a 1968 Western Pacific deployment in USS AMERICA CCVA 665. He was then selected for Naval Test Pilot School, - Patuxent River, Maryland from which he graduated in 1969. 5 . After serving two years as a Project Pilot with the Senfice 'ff' Test Division, Naval Air Test Center, Commander Lewis reported to Fighter Squadron ONE HUNDRED ONE for F-4 refresher training and subsequently reported to Fighter - Squadron FOURTEEN in July 1972. After two Mediterra- 1 nean deployments aboard USS JOHN F KENNEDY QCVA 671, Commander Lewis transitioned to the F-14A aircraft at 1 Naval Air Station, Miramar, California, while attached to l Fighter Squadron FOURTEEN, where he served as Oper- l ations Officer and Executive Officer. gl Commander Lewis then reported to Fighter squadron 1 ONE HUNDRED ONE in September 1975 and was as- signed as Officer-in-Charge of the F-14A Detachment for the establishment of the Atlantic Fleet F-14 Fleet Replace- ment Squadron. ln November 1976, he reported to Fighter Squadron ONE FORTY-TWO and served as Executive Officer until January 1978 when he assumed command of that squad- ron. After relinquishing command of Fighter Squadron ONE FORTY-TWO in March 1979, Commander Lewis reported to Commander Fighter Wing ONE where he served as the Chief of Staff. He served in this capacity until June 1980 -W' when he assumed command of Carrier Air Wing EIGHT. ln September 1981, Commander Lewis was relieved as Commander Carrier Air Wing EIGHT, and was transferred to M0 Commander Naval Air Force, U.S. Atlantic Fleet Staff, Naval , ,ss Air Station, Norfolk, Virginia. HOFFMAN U.S. NAVY COMMANDER CARRIER AIR WING EIGHT Captain David W. Hoffman was born in Philadelphia, Pennsylvania on March 18, 1941. He graduated from the U.S. Naval Academy in June 1962 and reported for Flight Training in Pensacola, Florida. After graduation and designation as a Naval Aviator, Captain Hoffman spent one year as a Flight Instructor in the Naval Air Training Command, after which he reported to Fighter Squadron ONE TWO ONE in San Diego, California for training in the F-4 Phantom aircraft. He completed two combat deployments in USS ENTERPRISE CCVAN 655 with Fighter Squadron NINETY-SIX before returning to Fighter Squadron ONE TWO ONE as the Senior Landing Signal Officer and Tactics Phase Leader flying the F-4 Phantom and A-4 Skyhawk aircraft. Upon completion of his tour, he reported to the Naval Postgraduate School, Monterey, Cali- fornia where he completed a Master's Degree in Operations Research in 1971. Another brief tour as an instructor in Fighter Squadron ONE TWO ONE was followed by his assignment to the Staff of Commander, Carrier Air Wing FIFTEEN as the Landing Signals Officer. While attached to that Staff, flying from USS CORAL SEA QCVA 435 on December 30, 1971, Captain Hoffman was shot down and captured near Vinh, North Vietnam. After his repatriation in March 1973, he became the first returning prisoner of war to deploy with a fleet squadron, making a Mediterranean cruise with Fighter Squadron ONE FORTY-TWO in USS AMERICA QCVA 665 in January 1974. Captain Hoffman transitioned to F-14's with Fighter Squadron ONE FORTY-TWO and served with the Ghost- riders until January 1976, when he reported to the Staff of Commander Fighter Wing ONE as F-14 Readiness Officer and then as Fighter Readiness Officer. He reported to Fighter Squadron THIRTY-THREE as Executive Officer on May 1, 1977, and was transferred to Fighter Squadron FORTY-ONE as Executive Officer in January 1978. He be- came Commanding Officer of the Black Aces from April 28, 1978 until June 15, 1979. In July 1979, Captain Hoffman reported to the Office of the Chief of Naval Operations QOP-9625 as a Strike Warfare Analyst. Later in the same year he became a Plank Owner inthe Anti-Air Warfare Division of OPNAV IOP-9555 as Fight- er Programs Branch Head. Captain Hoffman then became Commander Carrier Air Wing EIGHT in September 1981. 316 Captain Hoffman's personal decorations and awards include: The Distinguished Flying Cross, the Bronze Star Medal with Combat V and Gold Star in Lieu of his second award, 13 Strike Flight and one individual Air Medal, the Purple Heart with Gold Stars in Lieu of second and third awards, the Navy Commenda- tion Medal with Combat V , the Combat Action Ribbon and several campaign decorations. ' Captain Hoffman is married to the former Mary Burkhardt of Chicago, Illinois, and they have a son, David. They currently reside in Virginia Beach, Virginia. is if 'asm ,4 l-4?
