The Navy immediately rescued the other unhurt and searched all night for the missing man. A Barber ' s Point spokes- man said he could give no details on the accident and could not release the names of the two. The men went over the side at 4:18 p.m. and today the water in the area was 78 degrees and the seas calm. The Kitty Hawk had been und ergoing operational pre- paredness tests and was to have docked at Pearl Har- bor this morning. Accompanying the ship 4f e tJvD and helping in the search are three other Navy war- ships: the Chicago, Mahan and Arnold J. Isbell. . Lost Sailor Identified PA 7 A Navy man who fell off the carrier Kitty Hawk 130 miles at sea off Oahu and drowned has been identified as aviation electrician 3 Cl Cedric B. Boe of Greenwood, Wise. His body was not found. He was with helicopter composite squadron 1. He toppled off the carrier as it made a sharp turn to pick up another man who had fallen off a minute earlier. OCT tS ' - Vc A K p I 2 Sailors Fall, Only 1 Is Saved The Navy has called off the search for a helicopter crewman who fell off the aircraft carrier Kitty Hawk 130 miles at sea Monday night. The carrier, here lor op- erational readiness trials prior to Viet Nam duty, docked at Pearl Harbor yes- terday. Some 2,500 sailors with searchlights looked for the lost man through to dawn. A second sailor who fell in the water at the same time was picked up by a lifeboat immediately a n d was not injured. He was identified as 2 CI. aviation electronics techni- cian WiUiam D. Cook, an eight-year Navy veteran. Cook was the first on to fall. He was standing on the ship ' s forward elevator which brings planes up from the hangar deck to the flight deck. The ship made a turn just as he stepped back, and he toppled overboard. When I first hit the wa- ter, I began to swim away from the ship, Cook said. I started to shout, and I guess I did a pretty good job of it because they heard me all the way up on the flight deck (some 85 feet above the water). When the man over- board warning was given, the ship ' s skipper ordered a hard turn so Cook wouldn ' t get caught in the screws. The hard turn tossed the helicopter crewman off the ' other side of the canier. He is presumed drowned, and his name was withheld j pending notification of next ' of kin. AIRDALE rjMMANI)IN(J (►FFICER CAPTAIN M. 1 . :AI{M(H V Captain Martin D. Carmody was born in Indiana Harbor, Indiana on October 27, 1917. He was raised in San Jose, California and received his B.A. in 1940 from San Jose Stale College. He enlisted in the U.S. Naval Reserve in March 1941. Upon completion of flight training as an Aviation Cadet at NAS Corpus Christi, Texas, CAPT Carmody was commissioned as Ensign and designated a Naval Aviator on October 14, 1941. His tirst assignment as a Naval Officer was with Scouting Squadron 10 on the carrier USS ENTERPRISE. While with this squadron he flew as a dive bomber pilot, participating in the battles of Santa Cruz, Guadalcanal and Renell Island. In July 1943. he became Executive Officer of Bombing Squadron Eight on the USS BUNKER HILL and took part in every major carrier operation from the strikes on Palau Islands in March 1944, to Okinawa in December 1944, completing 53 combat sorties. In 1945, CAPT Carmody was assigned to Bombing Squadron 98 as Execulise Officer, lat er becoming Commanding OfTicer of that squadron. CAPT Carmody was ordered to the stafT of Commander Carrier Division THREE in November 1946, and remained as assistant Operations Officer until July I94S. In July 1949. following a tour as a student at General Line School in Newport. Rhode Island he was assigned as an instructor at the NROTC Unit at the University of Louisville. CAPT Carmody s next assignment came in July 1951. when he was ordered to .Airborne Earlv Warning Squadron 1 1, as Operations Officer until July 1952. He next commanded Fighter Squadron 124 and deploved with his squadron to the Korean War zone, participating in 63 combat sorties from the carrier USS ORISKANY until May 1953. CAPT Carmody then recei ed orders to the Office of the Chief of Naval Operations as head of the Attack Aircraft Branch of the Air Warfare Division. In this capacity he had much to do wit h the development and procurement of advanced attack aircraft embarked on our carriers today. From July 1955. until July 1957, CAPT Carmody was on the stafTof Commander Fleet Air Alameda as Operations and Training Officer. C. PT Carmody s next tour of duty was in the Atlantic Fleet as Commander Carrier Air Group EIGHT. This Air Group, while under his command, served aboard the USS RANGER and USS FORRESTAL. In July 1958, he was ordered to return to the Office cf the Chief of Na al Operations, to serve as head of the Fleet Training Section. CAPT Carmody remained in Washington until January 1960, when he was assiened as Executive Officer of the USS ORISKANY which subsequently deployed to the Western Pacific with the U.S. Seventh Fleet. Following this tour he was reassigned to Washington for duty in the Bureau of Naval Weapons, as Deputy Director Air Strike Warfare, and in 1962, he returned to CNO as Director Plannina Requirements Branch for Aircraft. CAPT Carmody was next assigned as Commanding Officer of the USS ZELLVLA ( AF-49) in January 1964. While under his command. ZELIMA made two tours to the Far East, servicing ships of the Seventh Fleet. After a brief tour with Commander Naval .Air Forces, Pacific, CAPT Carmody assumed command of the USS KITTY HAWK on May 29, 1965. and became KITTY HAWK ' s fifth Commanding Officer. CAPT Carmody has two sons, Russ. 19. and Bert, 15. who reside in San Jose, Cahfornia. During his career. Capt. Carmody has been awarded the Distinguished Flying Cross with two Gold Stars: the Bronze Star Medal with Combat V ; the Air Medal with eight Gold Stars and the Presidential Unit Citation Ribbon with three stars. Other medals and awards include: The .American Defense Service Medal; American Campaign Medal; Asiatic-Pacific Campaign Medal with two Silver Stars: the World War II Victory Medal; National Defense Sevice Medal; Korean Service Medal; United Nations Service Medal; the Philippine Liberation Ribbon and the Armed Forces E.xpeditionary Medal for active ser ice in the Vietnamese Crisis. He also wears the Korean Presidential Unit Citation Badge, as well as the Presidential Unit Citations awarded while serving with the USS ENTERPRISE the First Marine Division and the USS BUNKER HILL. ■« ' EXECUTITE OFFICER CDR W.F. McCULLOUGH Commander McCullough was born in New York City January 4, 1922. He joined the Navy July 15, 1942 as an Aviation Cadet and took pre-flight training at Troy, New York, and Cliapel Hill, North Carolina. Upon com- pletion of flight training at Corpus Christi, Texas, on July 15, 1944, he was commissioned as Ensign and designated a Naval Aviator. His next stop was Vero Beach, California and Night Fighter operational training. From there he went to Night Fighter Squadron 55, flying F6F-5N Hellcats. When World War II ended, he was assigned to Night Fighter Squadron 90 for his first peace-time duty. His next duty was with Night De clopment Squadron, At- lantic. July 1947 found CDR McCullough deployed in the Mediterranean aboard the USS LEYTE as a member o ' Night Composite Squadron 2 flying the Hellcat and the F7F Tigercat. Upon this return to the L ' nited States in 1948, CDR McCullough was assigned to the Tactical Test Division. NATC, Patuxent River. Maryland, as an all-weather project pilot. While there, he attended Test Pilot Training. In 1949. CDR McCullough became the first pilot to make a night intercept of a jet aircraft above 40.000 feet. In 1952. flvina F9F Panther jet fiahters with Fiahter Squadron 112 aboard the USS ' PHILIPPINE SEA, he won the Distinguished Flying Cross, Navy Commenda- tion. Air Medal and other awards for his combat missions o er Korea. Returning to the United States, CDR McCullough attended the University of California at Los Angeles under the Navy ' s Five Term Program. He also attended the Navy ' s General Line School and Aviation Electronics Officer school. His next operational duty was as Operations Officer flying the F3H Demon with All Weather Fiahter Squadron 53. In April, 1958, CDR McCullough was named Executive Officer of that squadron, and deploved aboard the USS TICONDEROGA in the Western Pacific in I960. He reported aboard the USS LEXINGTON in Febru- ary 1961 as Executive Officer of All Weather Fighter Squadron 213. In July. 1961. he became the squadron ' s Commanding Officer and was deploved aboard the USS HANCOCK in the Western Pacific ' until June 1962. He graduated from the Naval War College at Newport, Rhode Island in June 1963 and assumed command of Attack Squadron 127 in September 1963. He became Executive Officer of the USS KITTY HAWK on December 7. 1964. On Februarv 23, 1966. CDR Mc- Cullough left KITTY HAWK for dutv in Washington, D.C. . s , EXECUTIYE OFFICER Commander Eusene J. Carroll, Jr., is a veteran of 23 years of naval service. He entered the Navy at Los Angeles, California in July, 1943, as an Aviation Cadet. Completing flight training at Pensacola, Florida, he was awarded Navy Wings and commissioned as Ensign, United States Naval Reserve, on 10 April 1945. Appointed to the United States Navy in 1946, he has since served in widely varied assignments. Hiahliehts of Commander Carroll ' s career include four tours of duty in attack squadrons, the first of which included two Korean deployments as a member of Attack Squadron 65. He has served as Commanding Officer of two A4 Skyhawk jet squadrons aboard the USS SARATOGA (1962-1963) and Attack Squadron 43 at Naval Air Station Oceana (1964-1965) prior to reporting to KITTY HAWK on 1 June 1965. Commander Carroll holds four awards of the Air Medal for Korean action and has accumulated 5,3C0 hours of flight time and 446 carrier landings in Al and A4 aircraft. Other duties performed by Commander Carroll include a tour as Attack Squadron Flight Training Officer at Naval Air Station Los Alamitos; a cruise as Navigator of the USS GREENWICH BAY (AVP-41); 18 months at Naval Air Station Guantanamo Bay, Cuba; and 36 months in the Bureau of Naval Personnel. He is a gradu- ate of both the U.S. Naval War College (Command and Staff ' ) and the U.S. Army War College. Commander Carroll holds BA and MA degrees in International Relations from George Washington University. He is married to the former Margaret Steffen of Northumberland, Pennsylvania. Mrs. Carroll lives with their son Dennis in Coronado, California. Commander Carroll served as KITTY HA ' WK ' S Operations Officer from June 1965 until February 1966, assuming duties as Executive Officer of KITTY HAWK on 23 February 1966. In connection with these ASW operations, the then Commander Reedv received he Dis- tinguished Flying Cross. Bronze Star Medal, and the Air Medal decorations. He was also awarded the British Distinguished Flying Cross. Returning from overseas ' in ebruary 1945, he commanded Headquarters Squadron 5-- in Kev West, a night fighter training unit. The Naval Air Test Center at Patuxent River. Maryland was his assignment from December of 1945 until June 1947. His next assignments were as Air Officer and later Executive Officer. USS SHANGRILA followed by Operations Officer and Executive Officer aboard the USS PRINCETON. He then exchan ed his sea legs ' for a staff billet with Commander Naval Air Forces. Pacific Fleet. LACOUTIIRE Captain John Idwin Lacouture was born in West Ro bur . Massachusetts on August 18. 1917. After graduation from high school, he entered the United States Naval Academy in 1935. While at the Academy, Captain Locouture was active in a wide range of athletics. His hobbies today reflect a continuation of athletic interests, with squash, tennis and skiing among his favorites. He is also a member of the Florida Football Ollicials Association. CAPT Lacouture was graduated from the Naval .A.cademy in 1940 and was commissioned as Ensign on .Uine 6, 1940. Immediately following his commissioning, he was assigned to the USS SARATOGA as a Junior Division Olliccr. He was transferred in January, 1941, to the USS BLUE. While serving aboard BLUE, CAPT Lacouture was promoted to LTJG in June, 1942, and served as Assistant Engineering Officer, Communications Officer, First Lt. and Gunnery Offiicer. After the BLUE was sunk in September, 1942. he was transferred to NAS New Orleans for flight training in October and was pro- moted to Lieutenant. While stationed in New Orleans, he met his future wife, Elizabeth Monrose. They were married in New Orleans on October 26, 1943. The Lacoutures have two sons. John Jr. and Richard, and two daughters, Joan and Susan. After further training at NAS Pensacola, CAPT Lacouture was designated a Na al A iator and stationed at NAAS Melbourne as an instructor. In June, 1944, after one year at Melbourne, he became Executive Officer of VF-5. October 1944 brought a promotion to LCDR. followed by transfer in May. 1945, to VF-4 as its Com- manding Officer. Much of CAPT Lacouture s next three ears was spent as a student. From July 1947 until June 1949. he studied at NPGS in Annapolis, followed by three months as a test pilot at NATC Patuxent River, and nine months as a student at Princeton. He was awarded his MSE degree from Princeton in June. 1950. Since Julv. 1950. CAPT Lacouture has commanded VA-15, CVG-6, VA-43, and RCVG-4. He also served at NATC Patuxent River, Md., from January 1952 until October 1953. and spent two years in Washington D.C. in the Fighter Weapons Division of BUWEPS. In 1959. he was promoted to Captain and has served as SACLANT ' s European Representative and as Com- manding Officer of the USS DIAMOND HEAD (AO-19) and USS SARATOGA (CVA-60). His tour as Com- mander Carrier Division FIVE ' S Chief of Staff began in October 1964 and terminated in March, 1966. when CAPT Lacouture was transferred to Washinaton. CHIEF OF STAFF Captain James D. (Jig Dog) Ramage was born in Waterloo, Iowa, in 1916. He attended East Waterloo Public Schools and studied at Iowa State Teachers College for one year at nearby Cedar Falls. He was graduated from the Naval Academy at Annapolis in 1939. Captain Ramage prides himself as a carrier man, having commanded several carrier based squadrons. Bomber Squadron TEN (VB-10) aboard USS ENTERPRISE (CV-6) was his first command in 1943-1944. The sqaudron flew thirty-six SBD Dauntless Divebombers. In late 1944 he formed VB-98, which was the fleet replace- nient training squadron for SB2C Hell Divers. He was relieved by CAPT CARMODY in 1946. In 1952 he was ordered to command Carrier Air Group NINETEEN which operated in the Sea of Japan off Korea in USS ORISKANY (CVA-34). Two years later he commanded Composite Squadron THREE (VC-3) at NAS Moff ett Field, which provided night fighters to Pacific Fleet CVA ' s. In CVG-19 and VC-3 he flew the F2H-3 Banshee. In 1958 he relieved RADM REEDY (then CAPT) as Commander Heavy Attack Wing ONE at Sanford, Florida. Hat Wing -1 was composed of seven twelve plane A3D squadrons which provided heavy attack squadrons to the Atlantic Fleet. His venture into the patrol plane Navy occurred in 1960 as Commandmg Officer of USS SALISBURY SOUND (AV-13), flagship of Taiwan Patrol Force (CTF-72). He skippered USS INDEPENDENCE (CVA-62) m 1963-1964. Post graduate education includes Naval War College, 1947; National War College, 1958: and a masters degree in International Relations at George Washington Uni- versity in 1963. Shore duty assignments have included Chief of Sea Based Planning Division in the Office of the DCNO (Air); Chief of Nuclear Weapons Plans in the Office of the DCNO (Policies and Plans); and his most recent assignment as Test Director for the low altitude penetration testing of all U.S. tactical and strategic air- craft weapons systems, and the defences against such systems. His collateral duty was Director of Analysis and Reports for Joint Task Force TWO (JTF-2). Captain Ramage states, Carrier Air Power is just coming into its true position. KITTY HAWK is an out- standing example of American skill, and is manned by officers and men that are a credit to the United States. 1 am proud to be aboard in CTF-77. Williams on 31 December 1965, CDR Laws entered the Navv in 1942 and was designated a Naval Aviator in November 1944. CDR Laws duty assignments have included lours with VF-16A. VF-152 and combat operations with VF-64 in Korea. From 1954-1955 he was assigned to exchange duty with the 436th Fighter Dav Squadron. USAF, while this squadron was introducing the F-lOO Super Sabre, and participated in its operational evaluation. He graduated from the Naval War College with the class of 1957-58 and subsequent assignments included duty with VA-112 and as Commanding Officer of VA-22 during 1961-1962. Operational training in the F6F Hellcat and carrier qualification on the Old R.ANGER prepared him for his first squadron assignment. He joined VBF-81 on the USS PRINCETON at Norfolk, and sailed to the Pacific serving also in Fighter Squadrons 98 and 131. With Air Group THIRTEEN in 1949 he changed home port to NAS Jacksonville. Fla. In 1951-1954. on the next sea duty tour. CDR Williams served with Fiahter Squadrons 781 and 121 flying from the USS ORISKANY and USS BOXER in the Korean Conflict. Tn 1954 as an Air Force exchange pilot he served with the 3599th Fighter Squadron and at the Air Force Fighter Weapons School. In 1956 he headed the FJ Fury gunnery and tactics training at the Fleet Air Gunnery Unit. El Centro. CDR Williams has attended the Naval Line School. Monterrey, and the .Armed Forces StaiT College.
