CADUCEUS 1987 Indiana University School Of Medicine INDY Indianapolis was the back-drop for our educational experiences at Indiana University School of Medicine. Some of us had four years to experience the city, while others had only two. And for those who grew up here, it was simply “home. There is no doubt that the night spots were the most popular attrac- tions. We needed places to vent the frustrations and hostilities that built up during those long days. The beginning years saw us in Broad Ripple and Cast- leton frequenting such spots as the Vogue. Rosa Carona’s. TGI Fri- day's. The Corner Wine Bar. and Pawnbrokers.” Although these spots were and are still popular, a new scene emerged that was a little closer to the medical cen- ter. The revitalization of the downtown area offered many new places to relax. Names that will live in infamy in- clude. Sports. Ike Jonesey's. Shaboom's. the Elbow Room.” and The Common Market.” The new downtown also provided us with a multitude non-party places to spend our free time.” The opening of Union Station was probably one of the best things that could have happened. Over half of us were lucky enough to spend all four glorious years there, while the other half spent only two. Who could forget those nice, plush, comfortable chairs in MS 326? Many of us still sport scars from blisters acquired there from wig- gling to find a comfortable position. Those first few months were spent learning the other students' names that were next to you in the alphabet. Any hope of finding a spare moment of free time was quickly snuffed out by Micro and Biochem. We soon learned that Dr. Bauer did indeed create apathy! Anatomy. Histo. Physio. Neuroana- tomy and the ever-popular Psych soon followed. The philosophy of P = MD was very prevalent. There were those who moved into MERP or Butler Library for the duration. Class notes made this way of studying very tempting. Intra- murals added a great outlet for pent- up emotions. Right: All work and no play makes Jack a very thin boy!! Below: Chiel Complaint: splitting headache! Below Right: I still wanna fly jets!” Sophomore year saw a move to that bastion of comfort — Emerson Hall. The padded chairs were almost too much to take! The padding made it all the more difficult to remain awake through Pharmacology. Path, and In- troduction to Medicine. The dog labs stirred up controversy between stu- dents and faculty and also between students themselves. We also got a peek at the wards — History-taking and Physical Diagnosis. Some took seeing real patients as a shock while others were naturals at it. Junior year — need we say more? This offered all of us the opportunity to actually “care” for patients. The fields of Medicine. OB GYN. Psych. Neuro. Peds, Surgery, and the Surgical Sub- specialties were all explored. To many students’ dismay, however, this oc- cured to varying degrees. For some it offered numerous rewarding experi- ences while for others it was a never- ending battle to keep their heads above water. “Fear of the peer. as well as fear of the Resident and the Staff often got in the way of finding enough time to read and actually learn the art of medicine. We thought we had acquired enough knowledge on which to base our selection of senior electives and an eventual career. Senior year was definitely a long- awaited relief from the first two years, and especially junior year! Some have likened it to finding an oasis in the Sa- hara. Most of us spent senior year find- ing out that we didn't really want to do what we thought we did. or that we wanted some variation on a theme. The Dean's Office was kept busy with change of elective forms. We finally be- gan to believe that it was all worth it and prepared to go out and face the real world! There were few people in the Class of 1987 who found it difficult to have a good time. As we have said before, the night spots were always a good place to find members of the class. The Vogue was a frequented nightclub due to groups such as “Duke Tomato and his all-star frogs and “The Henry Lee Summer Band. Let js not forget the infamous Dr. Bop who performed at many of the school dances much to the dismay of the owners of the establishments (i.e. Mu- rat Shrine). Class parties like the Halloween par- ty were always big hits. The costumes were always unique and certainly never boring! The Christmas Dance was al- ways well-attended, especially by “Dr. Bop! While many of us went elsewhere for entertainment, a few of us dared to be different. Late during our sophomore year, a new group made its appear- ance on the Indianapolis scene. Put together from the best talent from around this great country of ours, the Kluver-Bucey Band arose to stardom! The name was so perfect — a little- known psychiatric disorder that drove its victims to derive sexual pleasure from inanimate objects! Who will ever forget the heart-stopping beat of drummer Rick Robertson, the titilating keyboards of Jamal Hakim, the melo- dious guitars of Ben Begley. You are the first ones I have watched as Dean throughout your entire experience here. Each of you has achieved more than you might have thought possible. Each of you is now prepared to move on to your next level of development with confidence. You have a great opportunity and responsibility to excel in whatever field of medicine you desire, and the future will need the best you can give — to patients, to each other and to society. New physicians of this decade will have a chance and an obligation to guide society through more difficult choices than any other group of the past or perhaps the future. Consequently, do not hesitate to engage in the debates society will have. You futures, the future course of the profession, and the future course of society depend upon the wisdom you will bring to the solution of the ethical issues facing us all — some known, some yet unknown. You have worked hard; keep it up; it’s worth the effort. The School wishes you well, I wish you well. Sci. lecture hall, room 326 hungrily awaiting, with pens in hand, tor those first words of Biochem to be unleashed upon us. Residency was a lifetime away. Learning the volume of material that lay before us was so all-consuming that sometimes simply surviving precluded thinking about the future. And yet somehow, the future is now. Graduation now over, we have survived, and we have friends and memories that will last a lifetime. Doesn’t it sort of amaze you to think of some of your friends as attending physicians in the next few years! So what can I say about wine that has anything to do with all of this? One of my favorite wine experiences is the opportunity to enjoy a very old bottle of wine. For me it is like having a time machine; an opportunity to journey into the past and reflect on one’s own life and the world and be amazed by all that has passed while the wine was laying there waiting to be consumed by you. This experience is denied to most of us because old wines are either unavailable or too expensive. As my final Rick's Picks. I’d like to encourage you. the reader, to buy a bottle of wine today to be consumed ten years from now to celebrate your tenth anniversary of graduation from medical school. You will not only have a rare, expensive and hopefully outstanding bottle of wine, but you will have the opportunity to journey back into your past. You will remember medical school, your friends, and all you have gone through in completing your residency and developing your new fruitful practice. I know ten years from now seems like forever, but so did graduation, remember? The next question is. what to buy? Remember, most wines do not improve with age and if you buy the wrong wine, you can look forward to opening a bottle of brownish, oxidized vinegar, quite a reward for all your patience. I’ll make a few recommendations of best bets that will most likely reward your ten-year wait. Plan on spending as much as S25.00 for the bottle (come on. you're a doctor now) because wines with longer aging potential tend to cost more. My first choice would be the king of red wines — Cabernet Sauvignon. Almost any Cabernet from a top California producer will last ten years. Look especially at the Reserve Cabernets which last even longer. A few examples include: Caymus. Mondavi Regular Reserve. Beringer Reserve. Inglenook Reserve. Sterling Reserve. Kenwood Artist Label. Chateau Montelena. BV Reserve. Clos du Val. and others If you want French Cabernet, a good Bordeaux from the '81. '82. or '83 Vintage should hold well. Ask tor help on this one. since it will be important to buy from a reliable Grand Cru Classe Vineyard. A petite chateau wine may not hold up. There is a Chateau Lynch Rages 1981 in town for $15-20 which should be a solid player The aging potential of Pinot Noir. Zinfandei and some of the big dry white wines like Chardonnay are well known, but there is a great variability between years and producers, so I don't recommend gambling with these. You may prefer a sweet wine. The best of them have a great aging potential. Buy yourself a good Vintage Porto, a French Sauternes. or a sweet late harvest Riesling. Of course there are many other possibilities, and endless special cases, but if you select one of the examples I have given, the odds of your being disappointed in ten years will be minimized. The past four years, even with all of the pain, are four years I wouldn’t trade for anything. I wish each of you happiness, good health, and success m all you do. In Vino Veritas. 57 Becoming A Physician: Ideal Vs. Ordeal By: Pamela Eva Fadul, M.D., Class of 1987 I am pleased to inform you that the Committee on Admissions has voted to offer you a place in the freshman class of Indiana University School of Medicine ... I’m sure most of us remember the day we received our letter of acceptance into medical school — it was probably more memorable than what we were doing the day JFK was shot. It triggered many emotions — pride, relief, elation, eagerness to begin. Once classes started, though, many appeared subdued, apprehensive, and anxious. Why? The stressors inherent in medical education had begun to take effect. Would we be able to master the vast amount of data? How would we react to cadavers, to death? What would the competition be like among the nation’s top two percent? How much in debt would we be after four years? Those and many other questions hovered both consciously and subconsciously. Coping and “surviving would become of paramount importance. During the subsequent four years some students adapted, but many medical students across the nation became cynical, dejected, frightened, depressed, frustrated. Some flunked out or dropped out. the majority of whom were not academically incompetent but. rather, unable to cope with psychosocial and performance pressures. What exactly do students find so stressful? What methods, both adaptive and maladaptive, do they use to cope with those stressors? What support systems do they (or would they like to) utilize? Is there a need for interventional programs? How do students’ personalities and attitudes change during medical school? The paucity of recent research data in those and other similar areas prompted me to approach the late Dr. Nancy Roeske with the idea for a “Well- Being Questionnaire. Together with Dr. Elizabeth Bowman and Ms. Donna Clair, we developed three questionnaires (one for medical students, one for house staff, and one for spouses partners) and obtained permission from the IUPUI Committee on Protection of Human Subjects and the Executive Committee of the Medical School to distribute them on February 1. 1985. A considerable amount of raw data have been generated from the 1.248 returned questionnaires. In addition. 387 spouses partners completed questionnaires. The data are being analyzed in several ways. The late Dr. Roeske wrote a paper entitled “A Study of Anxiety Depression Symptoms and Adaptive Behaviors of Medical Students. House Staff, and Spouse Partners. which was edited and submitted for publication by Dr. Hugh Hendrie, Ms. Clair, and Harry Brittain after Dr. Roeske’s untimely death. Briefly, that paper concluded that a significantly higher proportion of women trainees reported symptoms of anxiety depression than men trainees (41% compared to 27%). This difference between men and women was most marked during residency training the proportion of men reporting anxiety depressive symptoms declined between medical school (33%) and residency (10%). No such decline occured with women (medical students 42%. residents 37%). The year of training appeared to have relatively little effect on the occurence of anxiety depression symptoms for either medical students or house staff. Marriage appeared to confer some protection against symptoms of anxiety depression for house staff but not for medical students. The five major adaptive behaviors of anxious depressed medical students and house staff, as reported in the paper cited above, are listed in Table 1. Coping mechanisms differed between men and women, with women demonstrating more support-seeking behavior, more excessive eating, more smoking, more seeing movies. Sexual activity was the only behavior reported more frequently by male medical students than females. A surprisingly high number of medical students (81.13% of the sample) and house staff (25,11% of the sample) reported having suicidal thoughts at least some of the time. Thirty-four percent of spouse partners of medical students and 24% of spouse partners of house staff reported anxiety depressive symptoms. I have been analyzing students’ responses to questions dealing with their major concerns, their perception of the medical center environment, their attitudes regarding faculty and physicians, and the changes in their personalities and plans during medical school. It is beyond the scope of this report to list and discuss all the results, but I will give you some of the highlights (complete results will be published at a later date). Table 2 summarizes the results obtained when students were asked to indicate their degree of concern regarding 12 items ranging from academic and clinical pressures to personal areas. Understandably, academic clinical pressures were of most concern to medical students. Over 50% were also very much concerned about the strain medical education places on personal relationships and family, and the problem of fatigue. Physical health, mental emotional health, and a feeling of isolation also seemed to concern a great number of students. It is interesting to see how the year of medical school training affects students’ major concerns. The perception of excessive competition, while only of major concern to 29% of all students, peaked in the sophomore year at 41%. The feeling of ambivalence about becoming a doctor peaked in the third year with 30% of juniors indicating they were very much concerned about it. Perceived lack of faculty support and psychological harassment intimidation became more of a concern as students progressed in training, peaking in the senior year at 33% and 35% respectively. Medical students' perceptions of the medical center environment were generally positive. Many students indicated that the climate was conducive to learning most of the time (59%). and that the program was responsive to the needs of women (52%) and minorities (47%). Of those who had obtained academic assistance (17% had strongly felt the need for assistance). 54% strongly recommended it to others. Nevertheless. 53% felt powerless to change the medical education process. Sixty-five percent of all students felt that it was very easy or easy to talk to other students about professional concerns; however, only 31 % could talk easily to other students about personal concerns. As would be expected, many students indicated that they had inadequate time for reflecting on school and their personal lives (45%). and 48% rarely or never had adequate time for non- medical activities. In addition. 37% resented the demands of medical school which took them away from their families. Responses to seven of the questions revealed some of the attitudes regarding faculty and physicians. Fifty-one percent of all students felt that the faculty were responsive to students’ needs. In addition. 37% indicated that the faculty usually provide good role models. Nevertheless, the great majority of students said that they found it difficult to talk to faculty members about personal (83%) and professional (54%) concerns, and fully 44% did not feel that there was a faculty advocate to whom they could go to if they had a complaint about medical school. With regard to physicians in general. 45% answered that their premedical ideas about physicians have changed due to their experiences in medical school, and 29% said that their respect for physicians has actually decreased. Many more topics need to be explored using the same data base. For example, it would perhaps be valuable to compare those who indicated that they were depressed or anxious much or most of the time with those who completed medical school and residency without signs of undue stress. Drug and alcohol abuse is another area of much interest. Information from the house staff and spouse partner questionnaires would also be examined more thoroughly. So. what have we gained so far from the Well-Being Questionnaire? First, and perhaps most important. I believe we have made an important effort to raise the consciousness of the students, residents, faculty, and significant others. Only after recognizing and defining the problem areas can we promote a psychologically and physically healthy way to approach and survive the rigors of such a demanding professional training program. Moreover, it is a well known fact that physicians have a much higher incidence than the general population of suicide, depression, drug abuse, drinking problems, and divorce. If we can promote well-being early in medical school, perhaps we can decrease the number of physicians who later become impaired. Finally, the results from the Well-Being Questionnaire are pertinent not only to Indiana University but to medical students, house staff, and medical educators throughout the nation. As you know. Indiana University is one of the largest medical schools in the United States and its demographic composition is very similiar to that of the other medical schools.
