After grades are totaled and divided by the number of academic subjects, points toward academic awards are assigned each student based on the foI lowing sea Ie: Ave. 3.5+ = 3 points Ave. 3.25+ = 2 po i nts Ave. 3.00+ = I point 3. When a student accumulates 6+ points they qualify for an honor patch. When a student accumulates 12+ points they qualify for an honors letter. Each six points thereafter, a student qualifies for an honors pin for their Iett er. SENIORS WIN OPEN CAMPUS PRIVILEGES PARENTAL CONSENT FORM FOR THE SENIOR OPEN CAMPUS LUNCH PROGRAM Dear Parent or Legal Guardian and Concerned Student: Your signatures telow will signify that you have read, understood and agreed to the following provisions of the Senior Open Campus Lunch Program: 1. Students will be considered eligible only if they are seniors and are passing three academic classes. 2. The School District reserves the right to withhold this privilege from any student whose past record would, in the sole judgment of the administration, sliow that student to be a poor candidate for this program. 3. To be allowed to participate in this program, a student must have an identification card, and this card must be modified with a special sticker that will be administered by a school official. A nominal fee may be charged to cover the cost of replacement cards or stickers. 4. Any student guilty of aiding ineligible students to leave the school will automatically become ineligible. 5. All students who participate in the program will be responsible to help supervise the program on a rotating basis. Failure to conply with the terms of this rotating supervision will result in the student's loss of the privilege. 6. Students eligible to participate in the program will be allowed to leave the canpus during their assigned lunch period. Leaving the canpus during any other part of the day will be considered truancy. 7. Eligible students will be allowed to drive any vehicle for which they are responsible. 8. The student and parent or legal guardian acknowledge that the senior open canpus lunch program is a privilege being granted the student by the School District. That by participating in this program, the student and parent or legal guardian acknowledge that the School District will not be responsible for any personal injury and or property damage sustained by or caused by the student while they are leaving, returning or after they have left the school grounds. The School District will also not be responsible for any personal injury and or property damage sustained by or caused by the student as a result of the student participating in the rotating supervision of this program. 9. The School District reserves the right to tenporarily suspend this privilege due to weather or any other adverse conditions. 10. The School District may at any time permanently revoke this privilege if it is deemed necessary. 120 11.
