The formidable depth of his knowledge is an endless source of amazement. One is constantly reminded of the respect given him by students and physicians of all levels, when he speaks, we listen, for we know we can depend on what he says. But such qualities are not rare at Duke, the rarity of Dr. Baylin is that he combines these assets with a personal warmth and integrity which make him a truly great teacher and not just another brilliant doctor. The beauty of his knowledge is not just in its quantity, but in his genuine desire to share it with others. With gratitude and respect we dedicate the 1969 Aesculpian to: DR. GEORGE BAYLIN. Bi lication BAYLIN George J. Baylin was born in Baltimore, Maryland fifty-eight years ago to Pauline and Maurice J. Baylin. His father, a Russian immigrant, was in the general practice of medicine in Baltimore. George Baylin had a good start, he says, with the three most important people in his life, his parents, his mentor, and his wife. The A.B. degree at Johns Hopkins University in 1931 and upon entering the Duke University School of Nledicine. Here he came under the scrutiny and guidance of Dr. Frances Swett, who was chairman of the department of Anatomy at that time. One of the most valuable things he learned from Dr. Swett was the necessity of introspection and total honesty with oneself. Dr. Baylin earned his M.D. degree from Duke in 1937. One year later he married Sarah Hartman. They now have three sons: Stephen, an intern in medicine at Duke, who plans a career in cardiology: Jonathan, who has a IVlaster's degree in urban planning, and Eric, a very gifted and accomplished artist who recently graduated Phi Beta Kappa. Dr. Baylin is currently professor of Radiology at Duke. He has also held positions in the Department of Anatomy. He is a member of Alpha Omega Alpha and Sigmi Xi, and has received the North Carolina Medical Society IVledal. He was voted the SANIA Golden Apple Award in 1965. Watching his Herculean efforts in the outpatient radiology department is made more impressive by the fact that he has found time to author over one hundred publications. 41' 4 if f fs fi X W2 ' THE USE OF LABORATORY RESULTS TO IMPROVE CLINICAL DIAGNOSIS A computer cannot examine a patient. No amount of laboratory work can substitute for clinical observations. Looking at the color of the mucous membranes before and after the hemoglobin tests have been reported will improve the physician's ability to determine the degree of anemia. By examining the patient's heart and lungs before and after X-ray films are available will improve the physician's ability to make emergency diagnoses without the help of the X-rays e.g. spontaneous pneumothorax atelectasis of one lung or fluid in the chest. Some diagnoses can be made from the physical examination and the X-rays which cannot be made with the X-ray alone. A diffuse bilateral disease of both lungs may be tuberculosis, bronchial pneumonia, sarcoidosis, silicosis or berylliosis. If numerous medium size rales are heard after an expiratory cough, the diagnosis is tubercloisis or bronchopneumonia. If no rales are heard, the diagnosis may be sarcoidosis, silicosis or berylliosis. A history of exposure will eliminate silicosis or berylliosis or leave the diagnosis sarciodosis. A pneumonic lesion with a cavity by X-ray may be tuberculosis or an abscess. If the sputum is foul smelling the diagnosis is abscess. A filling defect in the liver scan may be a tumor or an abscess. If the patient has fever and the liver is tender on palpation the diagnosis is abscess. David T. Smith Professor Emeritus Division of Microbiology Many of you know that I believe you should achieve worthwhile happiness throughout your entire trip through education, including, of course, undergraduate medical education, graduate medical education and your continuing medical education throughout the remainder of your life. Don't delay happiness until the awarding of the doctorate or until you receive Board certification or until the establishment of your own practice. These should be memorable landmarks along your trip and should be celebrated. In addition, you should enjoy each day throughout your entire trip the pleasures of doing, of serving, of planning and of building. The happiness that should attend the doing of your multitudinous daily tasks of service will overshadow the pleasures you will receive through honors bestowed on you. Happiness is complex and there are undoubtedly many possible paths. One that l recommend involves the expenditure of the extra effort required to excel. Everyone enjoys most the games in which he excels. Likewise the extra expertice which you may acquire in your daily work will be paralleled by increased happiness during your work periods. lVlay I wish for all of you all possible happiness in your trip through medical education and your medical career. J. E. Markee lVl.D. Former Chairman, Department of Anatomy Medicine is a service profession. The doctor is educated and trained to give services to people. People have the right to make demands on the doctor. Sick people have the right to make irrational demands on the doctor. By the terms of the bond, the doctor will always spend a large part of his waking hours in professional activities. If he believes all professional activity is work, he will work a long day and develop a feeling of self-pity. If he believes the interplay between himself and his patients is as interesting and entertaining as a good play, an excellent novel or a game of tennis, he will have a hard time separating work from play, and he will wake up each morning with a feeling of zest for the adventures of the day. I have always been grateful that, in the summer of 1928, Emory University Medical School accepted Eugene Stead, Jr. as a freshman student. I know of no other career which could have brought me so many satisfactions. Eugene A. Stead, Jr., M.D. Professor of Medicine Former Chairman of Department of Medicine i If- 'Nx 'HHH Obstetrics and Gynecology Postgraduate Fellowship in a Department of Obstetrics and Gynecology Cincluding Gynecic Endocrinologyj can be stimulating and rewarding. The Specialty is not a mechanical specialty as some observers suggest. ln it will be found intellectual and scientific stimulation and knowledge.'Any department, which is fulfilling its obligation to the Fellows, must give to them a sound and excellent clinical experience for the care of the daughters of Eve, and must have excellent coverage of the Specialties within the Specialty. This will insure that the Fellows do have opportunities to accept the responsibilities of excellent clinical care of the patients and also the incentive to examine and consider the problems of basic and applied science which the patients present. The Fellows must learn that in no field of Medicine can, or will, they encompass all knowledge to bring them to the genius status. Each Fellow must accept the fact that postgraduate education is a continuous process of learning how to continue, through his entire life, the learning process and the ability to think. With this process of continuing to learn there will be a development of the Medical Conscience. The conscience, once developed will force him to clarify what we do not know or comprehend. Perhaps Socrates was correct, when as thought, he noted, t'The unexamined life is not worth living. Bayard Carter Professor Emeritus Former Chairman Department of Obstetrics and Gynecology Greetings to the 1969 Graduates of the Duke University School of Medicine: I envy you and wish that I could start studying medicine all over again, even though I realize that there is so much more to be learned than when I started in 1913. However, I enjoyed my sketchy and probably inadequate medical course. From 1913 to 1914, as a Rhodes Scholar, I took all of the subjects of the first three years at the Oxford Medical School in one year. It was even more condensed than the new Duke curriculum. I completed Anatomy, Biochemistry, Physiology, Pharmacology, Pathology, Bacteriology, and Junior Medicine and Surgery. In 1914, the First World War began, so I joined a French Hospital near Paris, starting as an orderly, and finally became an Anesthetist. In 1915, a typhus epidemic started in Jergo-Slavia CSerbiaJ so several of us volunteered. Later in 1915, all British medical students were ordered to return to medical school. As a neutral, I was requested to travel to England via Italy, Austria, and Germany to gather information for the British. The trip was interesting. On my return to Oxford, I became Sir William Osler's intern, and at night tested typhoid-Paratyphoid vaccines for the British and American Armies. In 1916, I was admitted to the Senior Class at the Johns Hopkins Medical School, but six months later, our country entered the War so I received my M.D. in April - and returned to France with the American Army for two years. I have been studying medicine ever since. Wilbur C. Davison James B. Duke Emeritus Professor of Pediatrics An ancient Babylonian legend has it that Chaos was a female dragon named Tiamat who was vanquished, but not slain by the god Nlarduk. The victorious lVlarduk proceeded to bring order to all things and set the stars, fashioned the earth and planned man's affairs. The resourceful Chaos!Tiamat, however, periodically returns to challenge the ordered serenity as evidenced by upheavals, earthquakes, hurricanes, epidemic, starvation, disorder, tumult and dissent. The dragon is subdued again and again and though many look upon her as the essence of evil, the famous American artist Ben Shahn eloquently defends Chaos!Tiamat as the bold provocateur who bestirs some men to noble deeds. We in medicine know full well of the frequent thrusts of the intransigent dragon. We retaliate with courage and ingenuity and discover antibiotics, decipher genetic codes, develop sophisticated means of birth control, transplant organs and constantly devise better means of preserving the state of health. Yet, the population explosion threatens us, transplants are rejected, epidemics recur, organisms become resistant and, most significantly, the failure to develop and apply effective preventive methods and adequate health care persists. How are these challenges to be met? I have little doubt that regardless of the nature of the curriculum most scientific mysteries will be solved. But what of the social obligations of the profession? Needed are changes in attitude and emphasis, and creative concepts of medical care. Teacher-Learner experiences must become even more intimate, exciting and frequent. Skills, facts and technology must be exquisitely seasoned with knowledge and wisdom and greater balance among the disciplines is necessary as we teach, administer to the sick and pursue research. Closer personal contacts are needed to foster mutual trust and understanding that will permit controversy, dissent and agreement to flourish in an atmosphere devoid of fatuous self-pride and spurious omniscience. We in medicine have built a successful profession, but not, as yet a successful culture. Much depends on our ability to thwart the disruptive thrusts of the ubiquitous dragon.
