Copied from Ingvoldstad's personal records, by permission. Statistical reports of chaplain activities submitted to the Chief of Chaplains, Bureau of Naval Personnel. Normally individual chaplain's reports were combined into more in- clusive reports before being forwarded by supervisory chaplains en route. Questionnaire prepared by the then Historian of the Chap- lain Corps, Chaplain C. L. Drury, and distributed through- out the Corps in the spring of 1954. Each reply was num- bered as it came in, and the appropriate number placed by the chaplainls name in a master code. Thus it was possible to read each questionnaire without knowing the name of the chaplain who had supplied the answers, if anonymity were wanted, or, conversely, the chaplain's name could be obtained if needed. As is usual with questionnaires, some of the material furnished was of little or no use. On the other hand, a large amount of interesting and sometimes very important data has been derived therefrom.
