Indeed the decision to become a doctor is usually made without any real knowledge of the world of medicine. One can't really know which medical school is best, or even best suited to his needs, because one can't know a school until he is there. So we do the best we can, relying on short visits, slick public relations brochures, and the reactions of students already there. Once at a given school, inertia sets in and we try not to think that we might have done better. And now, four very short years later, we find ourselves apply- ing again. Applying for internships is again done blindly. Geogra- phy and the preiudices of your department chairman weigh heavily on your mind. ln the end, it is luck or fate that you end up at the right place for you. Most house staff officers seem to like progr0mS, no matter where they might be. Indeed, house staff are a different breed from medical students, who invariably find fault with their institutions. Maybe it's because house staff are too busy and l00 tired to be really unhappy. Their hospitals have plenty of faults, but their programs are inevitably good. Or maybe it's because medical students must pay S2700 a year tuition - for such a price, one is entitled to perfection. V l i l f Kwai-mwqp-.Q P sl an ii? .4 M Q . 1 l W Most of us weren't married when we arrived at the medical school, and thus had the experience of sharing a dormitory room. These rooms were minimally comfortable for one, for two - well, if they were hospital rooms, they would be con- sidered substandard in size and wouldn't qualify for Medi- caid. At the time, we managed, in retrospect, we don't know how. When we arrived, there weren't even any overhead lights. We studied Biochemistry and Physiology in the dark, for all the difference it made. The fluorescent lights were a big help when they arrived, and served us well aside from the blackout iust prior to the anatomy final. Rubin Hall is furnished in Dormitory Modern - that mar- velous 20th Century architectural style that combines function- ality with cinderblock sterility and budgetary acceptability. A little imagination could remedy the situation - cooking facili- ties, lounges, and study rooms on each floor, or maybe less heat in the rooms and more in the bathrooms. And the eleva- tors - they do all they can to make those at Bellevue and the VA look good . . . But alas, no money. Will we ever see a pool or indoor basketball court? Oh, that N.Y.U. were blessed with the endowment of a P.8.S. or a Harvard. lt's a good thing that we get a damn good education here. Some of us are married, but there are no suitable married student apartments. Maybe we were lucky and got a good deal, but for many it meant either formidable Manhattan rents or a long commute from the outlands. There is no villain in this situation. The school is short of funds, and a reasonable allocation of resources cannot at present accord student hous- ing a high priority. If only some genius, twenty years ago, had thought ahead. This problem, along with many others besetting the school, brings to mind an old saying, Lack of money is the root of all evil. We were thus provided with a perfect excuse for our ignorance. Universities in New York are particu- larly vulnerable as other institutions in more pastoral lor at least, more liveable urbani set- tings use the lure of the good life to win away star teachers. The departure of faculty, even at the rate we have seen here, is not a sign of a school in decline. What many peo- ple fail to realize is that those losses are often a reflection of how good N.Y.U.'s fac- ulty is dndlcontinues to be. Indeed, it is a compliment when other institutions seek out our staff. Still, it is unsettling when so many departments must rebuild at once. N.Y.U. will survive, its students will learn. Nothing more need about a building which when finished will require an extra year of renovations before it can be opened to patients. And as for those promises five years ago clinical work there, well . . . Soon one of the patients was brought into the delivery room, and the resident and I scrubbed and donned our gowns and gloves. My excitement peaked with each contraction, as I hoped that this would be the one that would propel the baby's head into my eager hands. I-lowever, contractions came and went, and the baby was making extremely slow progress in the final moments of his descent. By now the other expectant mother's time had arrived, and she was brought into the second delivery room. Since that patient was progressing very rap- idly, the resident went into the other delivery room, leaving me and a nurse's aide with my patient. While the resident was deeply entwined in perform- ing the delivery in the next room, it happened. Ivly patient's baby decided that now was the time to complete his journey. Ivliraculously another resi- dent appeared, but did not have time to put on a gown and gloves, and therefore, could not manu- ally assist me in my predicament. After ascertain- ing my complete lack of experience, he calmly informed me that I was about to do my first deliv- ery. One is apt to vvitness some of the ugliest, along with some of the most beautiful scenes on earth, all within the same day. The feeling of belonging that one develops, in this fantastic place, is an intense experience. l was recently involved in a case vvhere a rare genetic enzyme variant apparently contributed considerably to the evolution of an extremely complex clinical problem. Such cases, in vvhich some expertise in the research lab and at the bedside are indispensable, although not every- day, will keep my professional life especially exciting. But these two letters do not make us physicians. We are physicians only to the degree that we meet the responsibil- ities of the physician. The major area of responsibility of the physician is to the patient. Certainly, we must strive to become clinically profi- cient, but in our pursuit of this proficiency we must never lose sight of the patient as a human being. Of course, we would all like to cure as many people as possible. But it is all too easy, in our haste to see many patients, to tall into the trap of regarding the individual as some sort of machine that has been brought in to be repaired. For every action there is a reaction is more than a law of physics. A human being is distinguished from other ani- mals by his ability to think and feel in an emotional sense. We will not always be able to cure a patient, but we should always be able to comfort him. We must anticipate the reaction of the patient to what we say and do and act accordingly so as to put him at ease. We must not erect barriers between ourselves and our patients - barriers which make some patients afraid to ask questions because they would be considered stupid by the doctor. An illness should not be regarded as a mystery which only the doctor understands but, instead, should be explained in layman's terms to the patient. Above all, we should be gentle, patient, kind, understanding, and compassionate. For, as Hippocrates said, . . . where there is love of man, there is love of the art. -For some patients, though conscious that their condition is perilous, recover their health simply through their contentment with the goodness of the physician. When I arrived the new Bellevue was just a foundation and Dr. Thomas had just become Dean. I remember the nausea that I felt when I first stepped onto a Bellevue ward an shown a young girl with lupus. Since then l have amassed two bookshelves of lecture notes, tried my hand at research and even teaching, witnessed a grad- ual but perturbing exodus of renowned faculty from the school, and spent enough rewarding moments in Belle- vue to feel that I would rather be a patient there than at U.l-I. Of one thing, how- ever, l am certain, our institution is in a state of profound transition, groping awkwardly towards resolution of the bizarre contrasts in medical care seen at the various hospi- tal facilities which comprise the Medical Center. And it is clear that final success in this transition will require more than new buildings or administrative reorganization. The empty grey bulk of the New Bellevue stands as silent reminder of the inadequacy of token gestures and mere material reconstruction in achieving a genuine improve- ment in the delivery of medical services. What is, ln fact, required is a genuine interest in improving the human con- dition of both our society and our collective consciences. That motivation, hopefully, will come from far-sighted administrators and from the compassionate few of our fac- ulty who have been the.inspiring teachers, who have shown by splendid example, what the word physician is all about. Each of us was once upon a time a single cell invisible to the naked eye. ln a small corner of the invisible speck was a tiny bag of DNA consti- tuting the whole genetic blueprint of you or me or anyone else you care to think of. That is to say this tiny bag contained all the embryology, anatomy, histology, physiology, biochemistry, endocrinology, hematology, neurology, immunol- ogy, etc. necessary to create a full-fledged human being and lead him from embryonic development to senilic decline. ln short, this tiny bag contained essentially all of our medical knowledge, or to be less presumptuous, I should say, it contained that portion of it which is true, as well, of course, as that medical knowledge which is yet to be discovered. It is rather humbling to think that after centu- ries and decades of research and despite scores of specialties and subspecialties within the vari- ous medical fields, we have barely scratched the surface of this microscopic mound of knowledge. We are still groping for the etiology of entities so common and well known as diabetic vasculitis and atherosclerosis. We have not even begun to seriously think of such things as the exact neu- ronal pathways of love or the molecular sub- strates of improvisation and invention. lvlost ironic, of course, is that each of the trillions of cells in our body contains a full set of all the information that was used to create us, so that we are literally full of knowledge, but do not know how to get at it! To the cell, however, the information seems quite clear indeed, and the metamorphosis to a full-fledged organism appears no more than a simple game that the fertilized oocyte merrily plays. Somehow DNA gives rise to RNA, RNA serves to produce proteins, these find their assigned stations in life, and soon a symphOUY evolves to the heart's beat and the brain's wave . . . and to our utter amazement. With time - perhaps decades or centuries - .-, .,-,..L-.f -1. .',.4-1., 1. '1T'I ' .rxrmx::vmflrv'v'vfr+? --'ES-ff-Ff-2?? the process of our genesis will probably be eluci- dated and its steps will probably be taught to medical students just as we were taught ad nau- seum the steps of the Kreb's cycle. With time, we may conquer cancer and learn to reverse ather- osclerosis. We may even learn to arrest the aging process. One mystery, however, will never yield to our scientific inquiries and that is the mystery of our freedom of choice. The problem is that even though we start with component elements whose nature we can define and those whose whole behavior we can predict, we end up with a creature with a will of its own, totally unpredictable. Scientifically speak- ing, Athis is simply not acceptable. Science is a study of mechanistic processes - whether they be one-step chemical reactions or a very com- plex genetic formula resulting in a bee's meticu- lously programmed life cycle. Causes and results are tightly coupled, and so we expect the same experiment to produce the same results. It can- not be any other way. lf it could, there would be no laws of nature, no scientific knowledge to deduce. In such a system everything is predicta- ble, even mutation. To the scientist the capacity 'to predict is the index of validity of his theories. Everything is predictable - except for Man. l-le defies all rules and transcends all laws. He feels no restrictions and knows no limits. He is free - even while not completely detached from nature. Like other animals, Man does hear the various calls of nature. But only he is able to turn around and emphatically declare no. Thus insofar as his behavior is concerned all of our investigations into his physical nature are bound to lead to a dead end. We may be able to pinpoint the biochemical substrates of his instincts and drives. But what do we do when he flaunts his biochemistry and refuses to obey? At that point it would seem that we can only sit back and admire his great leap into infinity as he softly whispers: in G-D's image. M 54 Ill yi ?s,XY r 1, l .f JG -2, l l Violet Journal of Pretentious Pafholo y Case Report: Rapid Mental, and Physical Deterioration in a Twenty-five Year Old Medical Student We hereby report with interest a case of extremely rapid mental and physical deterioration in a- twenty-five year old medical student. The etiology, it has been theorized, was a combination of stress factors which included losing a close friend in physiology laboratory and discovering q long lost relative in anatomy laboratory. , Patient was first seen in the fall of l969, and was iudged, on routine physical examina- tion, to be within normal limits except for a g tendency to grin excessively QSEG-sl lFig. ll. No changes were noted until January, 1973, if at which time lFig. 21 a marked increase in hir- sutism was observed. Also remarkable is the unmistakable facies showing first signs of a vacuous gaze and suggestions of an underly- ing paranoid psychosis. Shortlythereafter, the deterioration became evident to all. Patient lost all ambition, and began complaining of tinnitus, the etiology of l which could not be elucidated lFig. 3l. Whether this was a reaction to, or a part of, the underlying pathol- ogy has not been determined to date. Of course, numerous attempts were made to arrest the deterioration. Eupho- romycin therapy lMedical Violet, l97ll as well as several rather unorthodox pro- cedures lFig. 8l were of no avail. Lead- ing scholars in Medicine were consulted. Fig. 7 All were of the same opinion lFig. 9l. Unfortunately, as of this writing, the process has continued relentlessly. It has, if anything, accelerated lFig. lOl. Note the total alopecia, ecchymoses, and the general feeling of helplessness exhibited by the patient. Also of note, however,is the feeling of increased intelligence somehow conveyed by the patient. Our personal experiences leave their indelible mark on us. A very few of those experi- ences are recorded on these pages - a base on which our private vision of life at N.Y.U. can stand. Some yearbooks are works of art - photographic or lit- erary masterpieces. The 1973 Medical Violet has no pre- tensions to greatness, it is certainly not all we would like it to be. lt leaves untapped the creative genius of our class. lt leaves much unsaid about ourselves as individuals. And much unsaid about sickness and death, about health care and the miracle of life, about people - our patients, our future. But even so, we feel the primary function of a year- book has been fulfilled. I The-key to a successful yearbook is nostalgia. And this ingredient is not missing from the 7973 Medical Wolet.
